REFUND REQUIRMENTS
To recieve your refund, send ALL of the following items.• UPC Code - The entire box or bottle will also work
• Original sales receipt- showing product purchased, date of purchase, and name and address of retailer.
• A letter detailing your dissatisfaction with the product.
• Photograph of your driver's license or other picture ID.
• Phone number with area code where you can be reached during office hours.
– A representative may contact you to discuss your request and/or make suggestions for future success.

All correspondence must be sent via certified US mail, postmarked within 30 days of date of purchase.
If all items listed above are not received, PROVEN will determine if a refund is applicable.
