REFUND REQUIRMENTSTo recieve your refund, send ALL of the following items.
• UPC Code - The entire box or bottle will also work
• Original sales receipt- showing product purchased, date of purchase, and name and address of retailer.
• A letter detailing your dissatisfaction with the product.
• Photograph of your driver's license or other picture ID.
• Phone number with area code where you can be reached during office hours.
– A representative may contact you to discuss your request and/or make suggestions for future success.
